Bellwoods
Centres for Community Living offers clients non-medical support
services, accommodation, and independent living education
through Supportive Housing and Outreach services and programs.
Support
services are delivered under the client's direction,
provide assistance with the activities of daily living, and
include:
bathing and dressing
toiletting
personal grooming
meal preparation
assistance with eating
homemaking (at some locations)
Outreach
Servicesoffer
pre-scheduled support services for
people living in the community. Services are provided
in the client's residence,
a place of employment, or an educational facility where
clients pursue a degree, certificate or diploma.
Services are subsidized by the Ministry of Health and Long-Term Care.
Supportive
Housing Servicesoffer
transitional and permanent accommodation in a range of
settings, all with 24-hour access to support services.
Rents are geared-to-income and services are
subsidized by the Ministry of Health and Long-Term Care.

Park House has thirty-two fully
accessible one-bedroom and bachlelor apartments. Fourteen
units are dedicated to our Community Connect Program and
eighteen for permanent accomodation.
1082 Dundas Street West, Toronto The
Dundas Project has thirty-one fully accessible one-bedroom apartments for thirty-one clients.

Summerhill Road, Etobicoke The
Mimico Project has twelve accessible
apartments integrated into the Mimico Cooperative Apartment
Project.
Transition and Independent
Living Programs maximize independence and quality of life and cover areas
related to an individual's safety and well-being, such as
safety, budgeting, communication, mobility and stress management.
Programs are delivered using individual and group format.
The
Community Connect Program
at Bellwoods
Park House facilitates the transition of individuals from hospitals and long-term care homes to their optimal living environment, through safe-living education and 24/7 access to personal support services. Clients apply to either the six month or fourteen month program.
an intensive training program in the activites of daily living, with program coordination, designed to enable the individual to live safely in an accessible home environment in the community and to prevent re-hospitalization
accomodates up to fourteen clients in bachelor and one-bedroom apartments
only those individuals currently in acute care, rehab, Complex Continuing Care hospitals or LTCH are eligible for this program
The Mobile Independent Living Program (MILE): The objectives of the Mile program are to assist individuals to leave hospital, maintain themselves in the community thereby preventing hospitalization, and sustain a healthy life span.
the program provides ongoing independence training and program coordination to maximize safety and independence in the community
training includes skill development in home and community activity
program coordination includes access to resources such as community services, accessible housing, personal care equipment and maintenance, medical and other professional assisstance and financial resources
Workshopsare
available to all people with physical disabilities
living in the greater Toronto area. They are free of charge
and
include
refreshments. Support service workers can be provided if
requested in advance. Registration is required. Topics
cover such areas as Communication, Nutrition and
Cooking, are reviewed annually and vary according to the
interests of participants. Featured topics over
the last few years, for example, have included the Ontario Disability
Support Program (ODSP),
Housing, Volunteer Work, Leisure and Recreation,
Cooking for One and Foodsmarts, Assistive Devices, and Safety.
