Bellwoods
Centres opened a pilot transition program, Community Connect, in April, 2009, with the primary objective of transitioning patients with physical disabilities from health care facilities or Long Term Care Home (LTCH) to home. Community Connect utilizes enhanced supportive housing programs and services that are focused on clients living safely in an accessible home environment.
Program Objective:

focused on ensuring the client's ability to live safely in an accessible home environment
conducted within a community supportive housing environment with 24/7 access to physical care services
individual client objectives are focused on needs-based safe transition requirements
program activity is focused on skill development as well as access to resources
partnerships may be used to enhance supports for special groups such as seniors
For clients leaving a rehab program:
this program provides an extension of the person's rehab program in a community supportive housing environment
Target Clientele:
health care facility patients who do not need facility level service but need a transitional period in a supportive housing environment with 24/7 access to personal care services to ensure their ability to live safely in their own home or while waiting for LTCH placement
persons with physical disabilites including seniors fifty-five years and over
physical disabilites from neurological, stroke and musculoskeletal conditions
Program Admission Criteria:
Services:
persons sixteen years of age and older with physical disabilities
insured under the Health Insurance Act of Ontario
require personal support services in order to remain in the community
identified home to go to upon program completion and/or on LTCH waitlist
able to be left alone safely between service bookings
